 The binder that began this website in 2003
  • Personalized Medical  Binder!

    Materials needed:

    • 3 ring binder
    • A pkg of subject dividers or even better a package of 3M post it  brand index tabs

    Open the forms on your computer and customize them to your child. All forms open in microsoft word, simply print off the templates to create your own binder. 

     

     What do I put in my binder?

    Every child's binder will be individualized, based on who your child sees and how their program is set up.

    Using subject dividers (or those brilliant tabs you can label from post it, they are repositionable and reusable) divide up your binder into the following sections:

    • Yellow Pages: Print off an individual page for each person/professional your child meets with. You will have their info at your fingertips when you need it, not only for yourself, but for other doctors as well. Keep a record of appointments at the bottom of the page.
    •  Medical History: By filling out this form you will be ready to answer any questions a doctor may ask. Once a year check to see if any of the information needs to be updated.
    • Individual Doctors, Specialists, Groups....Using your handy subject dividers/tabs, set up a section for each doctor, therapist, organization that deals with your child. You will probably have a section for each of your yellow pages.
    • At the front of each section I keep a Record of Appointments sheet where I can jot down the basics at the appointment,( or even once my daughter is back in her car seat in the car before I forget.)
    • For each major appointment I take the time to fill out an Anecdotal Record Sheet. I do this once I am at home and have had time to collect my thoughts. I find it helps me clarify things since appointments are stressful. I also come up with questions that I need to ask.
    • Any questions I come up with I write down on a sheet of the Doctor Appointment Questions to Ask page. I never go to an appointment without them because I will inevitabley forget to ask something I really wanted to know.

    Any surveys or questionaires I fill out I have copied and keep in the relevant section. I often jot down when I sent it off, and when I got a response at the top of the page.

    The Form Filler at a Glance Template gives you a place to record ID numbers and Figures that you frequently need to put on forms so that you don't have to flip through your binder to find them. I keep this sheet at the very front of my binder.

    Any assessments I have are stored in the binder in the relevant section (speech, OT, School etc.) I often take out staples and use paper clips to hold each assessment together since I find different professionals often want a copy of everything. It is faster if you don't have to fight with staples.

    When I make appointments I ask what documentation they need. (If I can copy in advance, it simplifies things. I am less likely to forget or put it in an envelope and never mail it.)

    All documents should open in microsoft word so you can personalize them for your family.
